Nurses' Remuneration

This serves as a guide on the progression and promotion structures for government-employed nurses.

Ranks
Firstly, any person who is a citizen of South Africa intending to register for the first time as a professional nurse in terms of the Act, as having met the prescribed requirements to qualify as such, must perform remunerated community service for a period of one year. Thereafter, nurses receive notch increases of 3% every second year or 1.5% every year depending on the applicable grade.

The nursing sector is made of 3 main ranks, namely, professional nurses, staff nurses and nursing assistants. These ranks are further subdivided into the following grades:

Nursing Assistant 
  • Nursing Assistant Grades 1, 2 and 3 
Staff Nurse 
  • Staff Nurse Grades 1, 2 and 3 
Professional Nurse 
  • Professional Nurse Grades 1, 2 and 3 (General Nursing) 
  • Professional Nurse Grades 1 and 2 (Speciality Nursing) 
  • Clinical Nurse Practitioner Grades 1 and 2 (Primary Health Care) 
  • Lecturer Grades 1 and 2 

Promotions and Progressions
Promotion to each grade by means of grade progression is based on average performance and appropriate/recognised experience at each level. For each of the abovementioned grades, the appropriate or recognisable years of experience is as follows:

Nursing Assistant  

  • Grade 1 to 2: A combination of 10 years actual service and/or appropriate/recognisable experience. 
  • Grade 2 to 3: A combination of 20 years actual service and/or appropriate/recognisable experience. 

Staff Nurse  
  • Grade 1 to 2: A combination of 10 years actual service and/or appropriate/recognisable experience. 
  • Grade 2 to 3: A combination of 20 years actual service and/or appropriate/recognisable experience. 

Professional Nurse (General Nursing)  
  • Grade 1 to 2: A combination of 10 years actual service and/or appropriate/recognisable experience in General Nursing. 
  • Grade 2 to 3: A combination of 20 years actual service and/or appropriate/recognisable experience in General Nursing 

Professional Nurse (Specialty Nursing) 17
  • Grade 1 to 2: A period of 14 years actual service and/or appropriate/recognisable experience in General Nursing. At least 10 years of this period must be appropriate/recognisable experience in the specialty after obtaining the relevant 1-year post-basic qualification required for the relevant specialty. 

Operational Manager Nursing (General Unit)/Clinical Programme Coordinator 17
  • Grade 1 to 2: 10 years actual service as Operational Manager: Nursing (General Unit) Grade 1/Clinical Programme Coordinator Grade 1 

Clinical Nurse Practitioner (Primary Health Care) 17 
  • Grade 1 to 2: A period of 14 years actual service and/or appropriate/recognisable experience in General Nursing. At least 10 years of this period must be appropriate/recognisable experience in PHC after obtaining the relevant 1-year post-basic qualification required for the relevant specialty. 
Lecturer 17
  • Grade 1 to 2: A period of 14 years actual service and/or appropriate/recognisable experience in General Nursing. At least 10 years of this period must be appropriate/recognisable experience in Nursing Education after obtaining the relevant 1-year post-basic qualification.
